The TB Stigma Assessment Tool was created to help countries recognize why, when and where stigma happens, and understand the impact stigma has. By using the tool, countries and their national TB programs can be better equipped to develop targeted health and human rights actions to address stigma.

The Mental Health Assessment Tool was developed to bring awareness to behaviors that may potentially disrupt the TB treatment regimen. This tool includes a series of questionnaires and observations that can be documented during the patient’s medical evaluation.

This is a form for patients and case managers/health departments to sign indicating that they understand and agree to Directly Observed Therapy for treatment of TB.

This tool estimates the risk of active tuberculosis for an individual with a tuberculin skin test reaction of 5+mm, based on his/her clinical profile. It is intended for adults tested with standard tuberculin (5 TU PPDS, or 2 TU RT-23).

The New York City Health Code requires all health care providers to submit a written TB Treatment Plan to the New York City Department of Health & Mental Hygiene (DOHMH) within one month of starting treatment for newly diagnosed TB patients.
